These plans can be complex and continuous. These plans also include daily and long-term management goals, as well as how to handle emergencies.

Chronic illnesses and chronic conditions often require an influx in doctor visits and appointments. This influx is to ensure that we are monitoring the condition and altering the plan effectively to minimize the illnesses from progressing.
